Nurses and Nurse Managers

Certified Diabetes Educator
Sidney Hillman Family Practice
(Manhattan)/ Walton Family Health Center  (Bronx)

Requirements:          BA degree required. Nurse, dietician, physician assistant, social worker, health educator or other health professional certified by the National Certification Board for Diabetes Educators and have a minimum of three (3) years of clinical experience as a diabetes educator, preferably with a diverse population.  Must be able to work independently and must have good communication, problem-solving and counseling skills.   Bilingual in Spanish/English is preferred. 
           
Responsibilities:       Provide quality, comprehensive care for diabetic patients through individual consults and classes; collaborate with other team members in planning and delivering appropriate patient care; serve as a resource for physicians and other personnel in the care of diabetic patients; motivate and educate individuals with a wide range of abilities and backgrounds and vary instruction based on patient ability and motivation.

 


Licensed Practical Nurse
Urban Horizons Family Health Center (Bronx)

Requirements:          Experienced New York State Licensed Practical Nurse with Ambulatory Care, Medical/Surgical or Emergency Room experience preferred.  Bilingual Spanish required. 
           
Responsibilities:       Deliver quality nursing care to patients by assisting providers with diagnostic procedures and treatment regimens including performing venipuncture and administering immunizations in an ambulatory care setting as directed by the Nurse Manager.
