Medical Director
WeCARE Medical Assessment Unit

Requirements: Graduate of an LCME accredited medical school and AGME accredited residency program.  NY State Medical License; Board certified in Family Practice, Internal Medicine or Rehabilitation Medicine.  Current BCLS and DEA Certificate.  Minimum of one (1) year supervisory experience preferred.  Demonstrated effective interpersonal and conflict management skills, basic computer skills, ability to organize multiple tasks simultaneously and communication and organizational skills

Responsibilities:        Organize, delegate, and take responsibility for medical operations of the WeCARE Medical Evaluation Unit.  Hire, supervise and evaluate the Phase 1 Physicians.   In consultation with designated others, hires, supervises and evaluates Phase 2 Psychiatrists and Medical Unit Manager.  Organize, delegate, and takes responsibility for continuous quality improvement activities of the Medical Evaluation Unit to maximize efficiency and effectiveness.  Conduct case review including clinical rounds with FEGS Medical Director.  Collaborate with FEGS onsite managers Investigates and responds to patient complaints regarding quality of medical service.

 


Dentist (Part-Time)
Walton Family Health Center- Dental Department

Requirements: D.D.S. or D.M.D. from an accredited dental school; NYS License.  Minimum of two (2) years experience as a general dentist preferred. Demonstrated experience with dental electronic medical record preferred. Bi-lingual Spanish preferred. 

Responsibilities:        Provide direct and comprehensive care, including oral exams, treatment planning, diagnosis, x-ray interpretation, operative/restoration, removable and fixed prosthodontics, oral surgery, endodontics/ root canal therapy, periodontics, and preventive education. Refer patients to appropriate dental or medical specialties when indicated.   Obtain medical and nursing consultation when necessary in addressing total needs of patients. 

 


Family Nurse Practitioner
Homeless Center- Valley Lodge
                                                         
Requirements:
Current NY State Registered Nurse License and Registration; Nurse Practitioner Certification and Registration.  Minimum of two (2) years of nursing experience; minimum of one (1) year Nurse Practitioner.   Certified or eligible for certification by recognized specialty organization.  Bachelor’s degree and Master’s degree in Nursing required.  Graduate of an accredited Nurse Practitioner program.  Bilingual, Spanish preferred and interest in homeless population.     

Responsibilities:        Perform complete physical assessment; order and interpret diagnostic tests. Develop plan of care based on the diagnosis and individual client needs. Prescribe medications and treatments and make referrals to other disciplines.  Record, report, and interpret the client’s responses to plan of care. Teach, counsel and assist individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health. Collaborate with other members of the team in the provision of regular health screening and other health promotion measures.  Identify available community resources helpful to clients, refer and assist clients in utilizing such resources.
