Program Associate/Health Policy Analyst
Bronx Health REACH Program
Requirements: Masters degree in Public Health or Public Administration. 2- 5 years of related experience. Knowledge of health care disparities issues strongly preferred; detailed oriented, exceptional communication, organizational and interpersonal skills. Bilingual Spanish preferred.
Responsibilities: Develop and implement based primary prevention and health intervention programs for diabetes and cardio-vascular disease to effect lasting changes. Assist with Coalition building by identifying available community resources and develop linkages to support the primary prevention initiative. Train community members to assist with health education and outreach. Conduct research analysis on health-related legislation to support the Action Committee’s health care policy work. Work on grants as needed with the Planning and Development team.
Certified Diabetes Educator
Sidney Hillman Family Practice/ Walton Family Health Center
Requirements: BA degree required. Nurse, dietician, physician assistant, social worker, health educator or other health professional certified by the National Certification Board for Diabetes Educators and have a minimum of three (3) years of clinical experience as a diabetes educator, preferably with a diverse population. Must be able to work independently and must have good communication, problem-solving and counseling skills. Bilingual in Spanish/English is preferred.
Responsibilities: Provide quality, comprehensive care for diabetic patients through individual consults and classes; collaborate with other team members in planning and delivering appropriate patient care; serve as a resource for physicians and other personnel in the care of diabetic patients; motivate and educate individuals with a wide range of abilities and backgrounds and vary instruction based on patient ability and motivation.
AmeriCorps
Community HealthCorps Member
Requirements: High school diploma or GED or commitment to
obtain during the service year; United States
citizen, United States national, or lawful permanent
resident; at least 17 years of age; strong commitment
to service; respect for and interest in working
in medically underserved communities; excellent
people skills; ability to learn quickly; ability
to multi-task and meet deadlines; strong verbal
and written communication skills; creativity
and initiative.
Responsibilities: The Community
HealthCorps is a national AmeriCorps program
that promotes access to health care and health
information in medically underserved communities
while developing future healthcare workers.
Examples of HealthCorps placements available
at the Institute include: conducting one-on-one
and group health education sessions on preventive
health topics with patients at our health centers;
assisting health center staff to identify patients
who need help getting follow-up care and other
services as recommended by clinicians; and working
with Bronx Healthy Hearts to promote healthier
nutrition options in schools and restaurants.
We are currently recruiting
full-time members who serve for 1,700 hours
over an 11 month period. Members receive extensive
training and professional development, a living
allowance of $10,900 for the year, a $4,725
taxable education award to be used toward student
loans or future education costs, health insurance,
childcare benefits (if income eligible), loan
deferment on qualified student loans, and monthly
Metrocards.
