<p><strong>Requirements:</strong>  BS in Computer Information Systems or other related field or the equivalent of 5 years  of experience in a related area.  Experience in data gathering and analysis.  Familiarity with the healthcare environment, regulations and reporting  preferred.   Must possess strong analytical, interpersonal and organizational skills.  Must be able to communicate with patients and staff in a professional and congenial manner at all times.  Experience with SQL, SAP Crystal Reports and Business Intelligence platforms essential.  EPIC experience a major plus.  Must possess demonstrated superior analytical, design, development, testing, project management and customer service skills.</p>
<p><strong>Responsibilities:</strong>  In this role, you will facilitate data gathering and analysis from EPIC to provide comprehensive financial, clinical and productivity reporting throughout the organization.  You will work closely with users, including staff and patients.   Key responsibility is to ensure that all needs for reporting are addressed.  This includes validating reports supplied by EPIC, indentifying additional needs and producing a final product.  Will assist with project implementation as needs arise. </p>

<strong>Responsibilities:</strong> The Health Workforce Retraining Initiative is part of the New York State Department of Health’s Health Home model and supports the development and implementation of training that will prepare front desk staff, mid-level managers and care navigators to work in a patient-centered medical home service delivery model.  As part of this initiative, you will collaborate with key staff and other organizations to identify training needs and work with expert trainers to develop curriculum and assessment tools.  Other key responsibilities include scheduling and coordinating training, facilitating training as needed, managing the budget, assisting with program reports and continually reassessing and refining the program as needed.</p>

<p>Associate Program Director Job Description</p>
<p>Summary: Under the supervision of the Program Director, the role of the Associate Program Director is to develop, manage, coordinate and evaluate the Ulster County Healthy Start perinatal home visiting program. The Associate Director provides supervision to staff and technical support and direction to the Program Assistant. The Associate Director assists in the development and implementation of the outreach plan, and obtains necessary bi-directional agreements. The Associate Director is also responsible for reaching program and quality improvement goals and objectives, meeting Performance Targets, and reporting on results.</p>
<p>• Supervises Family Support Workers and other program staff; schedules and directs work, performs orientation, training, and quality assurance activities, recommends personnel actions such as selection, evaluation, promotion, etc. Identifies service, training and personnel needs within the program.<br />
• With Program Director, develops policies, procedures, and protocols necessary to carry out project activities.<br />
• Works with staff to assure maximum utilization of services by families and coordination among agencies within the target communities.<br />
• Assists Program Director with the preparation of budgets and quarterly reports.<br />
• Develops bi-directional agreements.<br />
• Coordinates activities with Advisory Committee.<br />
• Assures that all contract requirements are met. Assists in submission of program reports to funding sources. Responsible for monitoring and achievement of all contractual requirements and performance indicators.<br />
• With Program Director, explores funding opportunities and participates in research and development of grant proposals.<br />
• Responsible for consulting with Program Director on key decisions and keeping Program Director informed of program progress.<br />
• Coordinates with Supervisory Teams in Ulster and Dutchess Counties to capitalize on skills and create comprehensive services and systems across the sites<br />
• Performs other related duties as assigned including participation in Leadership and Management Teams in the organizations.</p>
<p>EDUCATION REQUIRED<br />
• Master’s Degree in Social Work or related field preferred, with a minimum of 5 years experience in Human Services in a Supervisory, Coordinator, or Leadership capacity.<br />
• Prior experience with at-risk families in health care or human services setting, management experience and quality improvement experience required.</p>

<strong>Responsibilities: </strong>Work as part of interdisciplinary team to help patients and families with personal and environmental difficulties, which predispose illness, of interfere with obtaining maximum benefits from medical care.  Prepare psychosocial assessments and patient histories, service plans and reports.  Provide case management services to assist patients in receiving necessary resources and services.  These include Medicaid, Child Health Plus, welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, mental health treatment, HIV services, legal, etc. Work with providers to assess child abuse and neglect and follow guidelines for reporting when necessary; provide follow-up support services to families to maximize family functioning.</p>
