<td colspan="4" valign="top" width="590"><strong>Summary: </strong>Serves as the lead point of contact for all HRIS systems functionality to support both short and long term goals of the organization.  The HRIS System Analyst will design, support and provide maintenance which includes conversions, upgrades, and database technology and data integrity.  Provide data management, complex reporting and confidentiality; supporting HR processes (i.e., performance evaluations, compensation, benefit enrollment, job requirements such as license, education, annual health assessment and annual training, etc).  Provide support and assistance to all system users, troubleshooting problems and interfacing with IT support and payroll.  Conduct regular audits to make sure data is current and correct as well as system cleaning.  Continuality analyst as needed.  Plan , implement and test system improvements and enhancements following best practice methodologies</p>

<li>Provide functional and technical expertise to support the HRIS system.</li>
</ul>

<li>Perform daily input of all HR data including, employee enrollment, changes and updates, compliance data.  Oversee HRIS data integrity and conducts system audits on a regular basis: maintain required documentation for external auditors.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Enter all data accurately into system regarding all new hires, employment status changes, HR compliance mandates.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Liaison for all leadership to assist with HRIS Manager-self-serve processes.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Liaison for all employees to assist with HRIS employee-self-serve processes.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Maintain a directory of standard reports for queries using appropriate reporting tools.  Provide reports as requested.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Provide support to the HR Team as it relates to HRIS systems needs.</li>
</ul>
<p>&nbsp;</p>
<ul>
<li>Provide update reports to payroll/finance to assure the payroll is completed with undated status changes information.</li>

<li>Download by system require data to feed into and support all HR related systems.  (Stromberg, Preview and Paychex).  Maintain accuracy of all data.</li>

<p>In addition to operating health centers, we use our expertise and resources to address <a href="http://www.institute2000.org/priority-areas/eliminating-health-disparities/">racial and ethnic disparities in health</a>, advance the use of <a href="http://www.institute2000.org/priority-areas/health-information-technology/">health information technology</a>, engage in research to improve health care services, and participate in policy development at the local, state and national levels. We also train health students and professionals at all levels through a number of programs including three family medicine residency programs, two free clinics that are operated in collaboration with medical students, the New York State Area Health Education Center System, (AHEC) Americorps/Community HealthCorps, and by placing students and volunteers at our health  centers and community-based program sites.</p>
<p><strong>Job Description</strong></p>
<p>&nbsp;</p>
<p>The Coordinator of Community HealthCorps and Student Placements will be a motivated self-starter who will have primary responsibility for the implementation of the Community HealthCorps program and for placing health professional students and volunteers at Institute health centers and program sites.  With grant funding from the National Association of Community Health Centers, the Institute has operated an Americorps/Community HealthCorps program for the past six years.  Each year, 15 members engage in a variety of projects that promote access to health care and health information in medically underserved communities.  In addition, the Institute assists medical, nursing, public health and other students gain exposure to community health by developing and coordinating clinical placements and internships throughout our network.  These initiatives are integral to the Institute’s commitment to training the next generation of allied health professionals to provide high quality, family-oriented care to people in underserved communities. </p>

<p><strong>Responsibilities</strong></p>
<p><strong>Community HealthCorps Program</strong></p>
<ul>
<li>Recruit, interview, select, and place members in projects throughout the organization.</li>
<li>Work with Institute staff from multiple disciplines to identify placements and develop  Healthcorps projects that support the Institute’s mission, and meet National Association of Community Health Center and HealthCorps requirements.</li>
<li>Develop and implement engaging and educational member trainings and meetings including: a five-day pre-service orientation at the beginning of each Healthcorps year; team meetings to promote group cohesiveness; and bimonthly trainings on health-related issues.</li>
<li>Maintain ongoing communication with all members’ supervisors.</li>
<li>Identify service volunteer events in New York City and coordinate member participation in these events throughout the year.</li>
<li>Ensure that all program requirements are being met including: tracking member hours, monitoring performance measures, maintaining member files, and monitoring program budget.</li>
<li>Serve as the primary liaison to the National Association of Community Health Centers for the Institute’s HealthCorps program, and represent the Institute’s Community HealthCorps program at regional, statewide and national meetings as required.</li>

<p><strong>Student Placements</strong></p>
<ul>
<li>Track all inquiries about placements. These include health professional students, volunteers, and students from AHEC programs.</li>
</ul>
<ul>
<li>Match students with placements and preceptors or supervisors throughout the Institute’s network.   This may include working with staff to develop appropriate clinical placements and internships.</li>
<li>Ensure that clinical placement agreements, human resources paperwork and AHEC forms are completed.</li>
<li>Work with New York Metro AHECs on program development and organize didactic sessions for AHEC summer programs.</li>
<li>Collect data and complete AHEC quarterly reports.</li>

<p><strong>Requirement and Skills</strong></p>

<li>Bachelor’s degree required, masters degree in public health or related field a plus</li>
<li>Two years minimum work/internship experience required, experience with volunteers and/or working with health professional students preferred</li>
<li>Interest in health care and human services a must; experience in these fields a plus</li>
<li>Strong communication, organizational, management, and group facilitation skills </li>
<li>Willingness and ability to travel to placement sites in Manhattan, the Bronx, and the Hudson Valley, grantee meetings, and annual conference</li>
<li>Team player; able to work well with multiple internal stakeholders</li>

<p>Reports to: Managing Director, Strategic Initiatives</p>
<p>Purpose of Position:</p>
<p>The lead nurse coordinator, working closely with a team of Institute providers and a health data analyst, will develop and implement a disciplined process to analyze individual patient and population data , as well as provider and practice data from all available sources with the  object of identifying patients with special needs, practice pattern anomalies among providers and practice sites that may impact the clinical outcomes and patient experience outcomes as well as cost of care. The lead nurse coordinator will use the analysis to work with the Care Management Group and others to stimulate and inform delivery system changes that will enhance patient health outcomes and patient experience and lower cost .</p>
<p>&nbsp;</p>
<p>Tasks:</p>
<p>Lead a team of clinical and administrative staff to</p>
<ol>
<li>Working with the health data analyst and a delivery system transformation team, develop a data analysis framework including identifying key analytical questions.</li>
<li>Analyze internal data available in the Institute’s electronic health record (EHR) system in combination with external data provided by health plans, the CHCANYS data warehouse and other sources to identify</li>
</ol>
<ul>
<li>high-cost, high-risk patients,</li>
<li>inefficient utilization patterns and their causes</li>
<li>opportunities for quality improvement;</li>
<li>opportunities for cost savings</li>
</ul>
<ol>
<li>Develop and apply patient survey and interview tools to identify the obstacles patients face to accessing services in the most efficient, effective manner.</li>
<li>Investigate and determine the causes of aberrant practice patterns, cost outliers and poor clinical and patient experience outcomes</li>
<li>Prepare/presentwritten and verbal reports for administration and clinical leadership including Care Management Group, summarizing analytical findings and recommendations regarding administrative and clinical interventionsand related delivery system changes </li>
<li>Work with clinical leadership to implement approved delivery system changes</li>
<li>Identify and examine inefficient workflows at specific practice sites, e.g., lack of provider-patient communication during care transitions between hospital and home</li>
<li>Develop actionable information for and incorporate this new information into the Institute’s comprehensive CQI process;</li>
<li>Track changes in clinical quality indicators, utilization patterns, and cost over time to measure the impact of delivery system changes</li>
<li>Participate in meetings with the health plans to review/analyze plan generated data</li>
